Office Policies

We appreciate your selection of our office to serve your dental health needs. Our goal is to provide the very best possible dental care for our patients so that each of you may achieve optimal dental health throughout your lifetime. Our entire staff operates as a team and we take great pride in each staff member’s training and capabilities.

Please review the following information, as you will be asked to sign and acknowledge the following policies at your first appointment.

Missed Appointments

When we reserve time for our patients we want you to feel cared for. That’s why we take such care in finding a time that will work for both of us. We know people are busy these days and sometimes need to make a change in their scheduled appointment. We can always find an appointment time for our busy patients if they give us plenty of notice, preferably a week or two. Unfortunately we cannot accommodate last minute changes due to our patients busy schedules. Please help us find a time where there is the least likelihood of change. We do need a four-day notice of any changes in order to keep your treatment warranties valid and current and to not lose any down payments made prior to treatment. Our broken appointment fee is $60.00. We place immense value in your time and hope you will honor us with the same in return.

Financial Policy

I understand that responsibility for payment for Dental Services provided in this office for myself, or my dependents is mine, due and payable at the time services are rendered unless financial arrangements are made in advance.  I understand that if my insurance company does not pay its portion of my balance in a timely fashion (within 60 days) the entire balance becomes my sole responsibility and is due immediately.  I must then collect any funds due from my insurance company myself.  I further understand that any balance unpaid after 90 days will be subject to reasonable collection costs and interest and sent to a collection agency for collection.

I understand that benefits quoted are not a guarantee of payment, only an estimate that was provided by your insurance company.  I authorize release of any information relating to these claims.  I authorize payment of the dental benefits otherwise payable to me directly to Terry D. Hopper, D.M.D.

Health Privacy

Our office adheres to government HIPAA regulations. A copy of our Compliance Notification is available upon request from the front desk.
